The following Terms and Conditions apply to the use of this Web site as well as all transactions conducted through the site.

• Consumer Agreement

• I, the undersigned, desire for the person named as student herein to participate in classes offered by the Performance Club at the above location.  I hereby voluntarily release the Performance Club, their staff and employees, LJ Woodard and instructors from any and all responsibility for injuries or problems of any nature that may arise as a result of the named student’s participation in this program/activity at the above location or any other performance venues. If injury occurs, it is understood that the student be covered by their own family policy.  I am aware that participating in the said program/activity may involve some strenuous physical activity and I state that my child is in the proper physical condition to participate.

   

  I hereby give permission for images of my child, captured during regular Performance Club classes, rehearsals, performances and special activities through video, photo and digital camera, to be used solely for the purposes Performance Club promotional material, newsletters, websites and publications, even the sale of DVDs from Performance Club Shows to parents and waive any rights of compensation or ownership thereto.  




• Return Policy

• Refunds:

  The Registration fee is non refundable. A full refund of tuition will only be given (less $5 administrative fee) if requested prior to the start of the program.  If requested after the program has started, 40% plus a pro-rated amount for all days already conducted will be assessed, whether or not the child was in attendance. Credit towards another class or program will be honored as well.

   

  Make Ups:

  Classes may be made up within the 8 week session.  Any class of similar level may be utilized as a make-up.
